Supporting People with Mental Health Issues
Our outreach service supports individuals with a mental wellbeing issue in their own home. The aim of the service mirrors the approach in our other services: to promote independence and integration using a recovery-based model.
The service operates Monday to Friday, and the number of hours provided is flexible and adaptive to meet the needs of the individual service user. During their visits, each service user can expect support to gain and maintain valuable life skills such as budgeting, shopping, cooking, bill paying and support to access meaningful activities. The aim being that in time the service user will have the confidence and ability to use these life skills independently of support from staff. The service is person-centred and is always service user-led allowing the service user to set aims and goals, deciding what support is required to achieve these.
Referrals to the service are made by Health or Social Work professionals.
